Support in Hospitals
Carers FIRST has staff based in Tunbridge Wells hospital and Darent Valley hospitals. This Service is available to support and provide carers with information when the person they care for is in hospital or when they are a patient themselves. We can liaise with hospital teams and social services to ensure you are able to access the correct support for you and the person you care for and support you to have conversations with the Hospital teams. If you, or the person you are looking after has a planned admission into hospital, please let us know as soon as you are able so we can offer support. For more infomation about the service available across hospitals in Kent and Medway, you can download our leaflet here.

Carer Hospital Discharge Service
If you need extra support when the person you care for is discharged from hospital a Carer Support Worker can discuss with you the type of help and support available to meet your needs. This may include practical support to help you in your caring role. This support is free for up to two weeks from discharge.

Please note that Carers registered with a Dartford, Gravesham, or Swanley GP Practice cannot access this service.

Carers Assessments

Under the Care Act 2014, if you are a Carer and you appear to have any level of needs for support, you are entitled to a Carer’s Assessment. Having a Carer’s Assessment is not linked in any way to Carer’s Allowance.

A Carers Assessment is the opportunity for you to tell  someone what impact caring has on your life. The assessment may be carried out on the telephone, in a community venue, or in some circumstances at your home. The assessment will look at how caring affects your life, including for example, physical, mental and emotional needs, and whether you are able or willing to carry on caring.

To find out more information, including how to prepare for your assessment, please see the Carers Assessment page on our website.
