Exploring your care options, or the help available to keep your independence, can be daunting – even stressful. To help you, Lincolnshire County Council have co-produced a Care Services Directory for 2017/18 with clear and easy to understand information for you to consider when thinking about care choices for yourself, your family, or someone you care for. 

In this publication, you’ll find information on staying independent at home, support for carers, activities in the community, your options for housing and care homes and financing your care.

The Lincolnshire Dementia Family Support Service (DFSS) help people with a dementia diagnosis, their families and carers. This countywide service is delivered by specialist Alzheimer’s Society Family Support Workers who help people living with dementia, their carers and family, to navigate through their journey of dementia ensuring service users can step up and step down from other sources of support they require. If appropriate Carers FIRST and DFSS will complete a joint visit to the carer to complete a carers assessment and support plan.

Lincolnshire Safeguarding Adults Board (LSAB) is a multi-agency partnership, comprising of a range of organisations that all have stakeholder interest in the safeguarding adults agenda.

The aim of Lincolnshire’s Safeguarding Adults Board is to ensure the effective co-ordination of services to safeguard and promote the welfare of adults, in accordance with the Care Act 2014 and Care and Support Statutory Guidance 2014.

The Board’s objective is to improve local safeguarding arrangements and ensure partnerships act to help and protect adults at risk of experiencing neglect or abuse.

Macmillan: They provide practical, medical and financial support and push for better cancer care.

Stroke Association: Their service provides high-quality information, practical advice and emotional support following a stroke. They begin working with a stroke survivor and their family immediately after a stroke and provide the support people need for as long as they need it – helping within their own homes and helping them back into the community.

If you have concerns or questions about any aspect of your care, or the service you receive, it is best to first speak with a member of staff involved with your/their care. If you find that staff cannot help you, or you are not comfortable speaking with them, you can contact Patient Advice and Liaison Service (PALS).

PAACT: (Parents and Autistic Children Together) We are a group of parents who have children with Autistic Spectrum Disorders.
