The Young Adult Carers (YAC) Transitions project provides a service to those caring for a relative, partner or friend who needs support because of disability, illness, addiction or fraility. The purpose of this service is to support young adult carers in their caring role and enable Carers to:
Have access to information, advice and guidance
Have access to integrated and personalised services
Have a life of your own
Have support to stay mentally and physically well
Promote your financial wellbeing
Have a voice about the services for the person they care for and for themselves to ensure young adult carers have a smooth transition into adulthood
Have support to access further education, training and employment
Opportunity to access Statutory Carers Assessment and associated support
Carers FIRSTs transition staff work within local schools and colleges to identify and support Young Adult Carers and to ensure that they have someone that they can talk to about the choices that they have before them. The transitions project offers 1-1 support alongside regularly run workshops and activities which provide young adult carers with the opportunity to meet others that are in caring roles, to develop friendships and gain from the support of experienced staff.
